Every dog is different, and thoughtful grooming begins with understanding their individual needs. Before your dog's first appointment with Fourfold Canine Wellness, please complete this intake form so Maiya can learn about their health, coat, grooming history, behavior, and comfort preferences.
A few things to know:
Please complete one form for each dog, even if multiple dogs belong to the same household.
Returning clients only need to complete this form when bringing a new dog or when an updated intake is requested.
Please provide accurate, current information and notify Fourfold of any important changes before future appointments.
This form does not replace the Grooming Consent & Safety Agreement or Social Media & Photo Release provided when securing your appointment.
